We also have one of the largest inventories of Chinese Patent formulas in the state. In order to prescribe out of our pharmacy we ask for you to down load the "Guidelines for Ordering" form. Please read and sign the form, then send it in with your initial order form.

As advocates for education, Formulations offers affordable healthcare classes in our medicinal herb garden, as well as advanced, CEU approved education for practitioners.
